To become a therapist in the state of Maine, you must complete the application process by submitting proof of the following:
Application with background check
High school diploma or the equivalent
A minimum of 500 hours in classroom and clinical instruction
Or
Received a pass grade on the MBLEx (Massage & Bodywork Licensing Examination)
Current copy of First Aid
Current copy of Adult CPR
A 3x5 current photo signed and dated
An $86.00 check, payable to: Maine State Treasurer. This is to cover the non-refundable $25.00 application fee, $40.00 initial license fee, and the $21.00 background check fee..
