Application for State Licensure- Fill out and send back a notarized copy to obtain your license. You must also obtain 2 fingerprint cards and fill out the background check (it takes up to 3-4 months to get results, so send it in as soon as possible).

The Nevada State Board of Massage requires you to send in an official transcript with 500 hours or more of an accredited school. They also need a copy of your scheduled National Certification Examination or the final results of your NCETM/NCETMB with your application in order to be eligible to obtain a license to practice Massage Therapy in the State of Nevada.
