Florida Licensure for Massage Therapy - Start here for questions about massage therapy licensure in Florida. Florida Massage Therapy License Application - Direct link to the massage therapy license application that you need to download, print, fill out, and submit. The Florida Board of Massage Therapy requires that you must be at least 18 years of age and have completed an approved massage course of at least 500 hours or an approved massage apprenticeship training program of at least 12 months. The state of Florida also requires a massage license applicant to take and pass a 2 hour prevention of medical errors course. Before a Florida massage license is issued, you must also pass the National Certification Examination for Massage Therapy NCETMB. The NCETMB will need to send your results directly to the Florida Quality Assurance Board of Massage Therapy. Date has changed due to settlement with NCBTMB, it may be August before applications will be accepted for the MBLEx by the Florida Massage Board. **** The MBLEx with the FSMTB will be the only exam for Licensure starting May 1st 2009.****
