The Florida Board of Massage Therapy requires that you must be at least 18 years of age and have completed an approved massage course of at least 500 hours or an approved massage apprenticeship training program of at least 12 months. The state of Florida also requires a massage license applicant to take and pass a 2 hour prevention of medical errors course.
Before a Florida massage license is issued, you must also pass the National Certification Examination for Massage Therapy NCETM/NCETMB or the MBLEx governed by the Federation of State Massage Therapy Boards (FSMTB). The NCETM/NCETMB and MBLEx will need to send your results directly to the Florida Quality Assurance Board of Massage Therapy.
Florida also accepts the BCTMB Advanced Certification exam. But you must have 250 Hrs. of post graduation Hands on experience- No matter the amount of school hours (6 mo. minumim Hands on Experience). You can not use the BCTMB to obtain a license, you must use the NCETM/NCETMB or the MBLEx to apply for a state license.

4- Criminal History and Health History Documentation if needed
