Application for Massage Therapists - Send in with application fee of $150. The State of Ohio Medical Board processes many applications for all Medical certificates to practice a limited branch of medicine like Massage Therapy. Applications are processed in the order in which they are received. There will be a delay in processing if the application is found to be incomplete or if there are any unusual circumstances that are discovered during the processing. The state will notify the applicant if there is an issue with the application. Before applying for a license, Ohio requires the applicant to have their Massage school certification in hand. Each Licensed Massage Therapist is required by Law to obtain a $25 Vendor's License from the County Auditor's Office or the Department of Taxation.
